Cancellation Policy

Mind Body Movement Living Laboratory (MBMLL) Participation, Liability & Media Release Operated by Providence Family Life Center (PFLC) 1. Assumption of Risk: I voluntarily participate in wellness activities (yoga, movement, breathwork, research) at MBMLL. I understand these involve physical exertion and potential risk of injury. I confirm I am physically/mentally fit to participate and will move at my own pace, stopping if I feel discomfort. 2. Medical Disclaimer: These programs are educational wellness services and not a substitute for medical or mental health therapy, diagnosis, or treatment. I will consult a healthcare provider for any medical concerns. 3. Waiver of Liability: To the fullest extent permitted by Florida law, I release PFLC, its staff, and affiliates from all liability for injuries or damages arising from my participation. 4. Media Release: I grant PFLC/MBMLL permission to use photos or recordings of my participation for educational, promotional, and grant-reporting purposes without compensation. 5. Cancellation Policy: Cancellations must be made 24 hours in advance to avoid charges. No-shows or cancellations within one hour of class are non-refundable. By signing, I acknowledge I have read and agree to these terms.
