Cancellation Policy

Mind Body Movement Living Laboratory Participation, Liability & Media Release Operated by Providence Family Life Center Corporation I voluntarily agree to participate in classes, programs, workshops, or research activities offered through the Mind Body Movement Living Laboratory operated by Providence Family Life Center. I understand that participation may include physical activities such as yoga, tai chi, breathwork, and other wellness or movement-based exercises. These activities involve physical exertion and may carry some risk of injury. By participating, I acknowledge that I am responsible for my own health and safety and that I am physically and mentally able to participate. I understand it is my responsibility to consult a healthcare provider before beginning any new exercise or wellness program if I have health concerns. I agree to participate at my own pace and stop any activity that causes discomfort or injury. I understand that programs offered through the Mind Body Movement Living Laboratory are educational wellness programs and are not a substitute for medical care, mental health treatment, therapy, or professional diagnosis. I agree to seek guidance from qualified healthcare providers for medical or mental health concerns. To the fullest extent permitted by Florida law, I release and hold harmless Providence Family Life Center Corporation, its staff, instructors, volunteers, and affiliates from liability for injuries, damages, or claims arising from my participation. I grant permission to Providence Family Life Center and the Mind Body Movement Living Laboratory to photograph or record my participation for educational, promotional, website, social media, and grant reporting purposes. I understand that no compensation will be provided for the use of these materials. Cancellation Policy: Cancellations made 24 hours or more before class will not be charged. Cancellations made less than 24 hours before class may be charged, and cancellations within one hour of the class start time or no-shows are non-refundable.
